High School Required Documents

The following documents are mandatory for all players in order to participate at the high school level. Athletes may not practice or compete until all required forms are completed and on file:

• Physical Form – Valid for 2 years
• iMpact Concussion Baseline Test – Valid for 2 years
• US Lacrosse Consent to Treat Form

Please ensure all documents are submitted and current prior to the start of the season.

Beginning July 1, 2025, USA Lacrosse requires all players to age verify their membership to be active. You can age verify now by logging into your USA Lacrosse account or you can submit documentation during the join/renew process. This is a free one-time process that will remain valid for the duration of a player's active USA Lacrosse membership
