Required Paperwork Information and Links to Forms for Fall & Spring Registrations (High School Players ONLY)

In order to complete registration for fall and spring, players will need to download the following, completed forms:

1) Current USA Lacrosse Membership (must renew every year)

All players are required to have a USA Lacrosse Membership valid through November 1, 2023 for Fall Ball and June 1, 2024 for Spring Lacrosse.

2) Health Examination and Consent Form (valid for 2 years)

Freshman, Juniors, and New Players will need to complete the sports physical (2 pages!) with their pediatrician, family practice doctor, or instacare center.  Date must be valid through November 1, 2023 for Fall Ball and June 1, 2024 for Spring Lacrosse.

3) Online Concussion Test (valid for 2 years)

Freshman, Juniors, and New Players are required to complete an online concussion test through ImPact Baseline Testing.  The cost is $20 and takes about 30 minutes to complete.  Download the certificate of completion with your player's name and test date on it.  Date must be valid through November 1, 2023 for Fall Ball and June 1, 2024 for Spring Lacrosse.
